The mission of Pine County Health & Human Services is to Promote and Protect the Health, Well-Being, and Self-Sufficiency of All.
Pine County provides case management services for eligible adults & children who have been diagnosed by a mental health professional with a severe and persistent mental illness.
Adult Mental Health Case Management
Services may assist in gaining access to needed services regarding: education, health, mental health, social, vocational, and other necessary services as they relate to the client's mental health needs.
Case managers will assist an individual in completing and/or monitoring:
- Functional assessments
- Individual community support plans
- Refer and assist the person to obtain needed mental health and other services
- Ensure coordination of services
- Delivery of services.
Children's Mental Health Case Management
Services are voluntary, and may be accepted or refused by the parents.
- Assist in obtaining needed services by coordinating with other agencies and assuring continuity of care for the child.
- Work together with the family to continually assess and reassess the delivery, appropriateness, and effectiveness of these services.
- Connect with the people involved in the care, counseling, education, and treatment of the child in an effort to coordinate and monitor services.
A variety of services available through Children’s Mental Health Case Management may be provided based on the individual needs of the child and family.
