The Adult Mental Health Initiative serving Chisago, Isanti, Kanabec, Mille Lacs & Pine Counties, and the Mille Lacs Band of Ojibwe is partnering with Wilder Foundation to conduct a regional mental health needs assessment to help improve our regional mental health resources.
Individuals/consumers completing the survey must live or work in Chisago, Isanti, Kanabec, Mille Lacs, or Pine County and are completing the consumer survey in a non-professional capacity. If you meet those requirements you will receive a $20 gift card to Walmart just for answering the questions!
